This is to certify that the health of Capt. Candler has been very precarious for the last twelve months, he is very much afflicted with cough, the effect of chronic inflammation of the lungs, which renders him unfit for camp life.

Be it Remembered, That on this Sixteenth day of July Anno Domini one thousand eight hundred and sixty two personally appeared before me, the subscriber, a Justice of the Peace, for the county aforesaid, John H. Candler and made oath on the Holy Evangelist of Almighty God, in due form, that the annexed account, amounting to Three hundred & sixty four Dollars and --- Cents, is accurate and just; that he has not received pay, drawn rations or forage or clothing in kind, or received money in lieu thereof, for or during any part of the time therein charged; that he actually employed and kept in service, the horse charged for the whole of the time charged; that his place of residence is Russell Co., Va and that he is not indebted or accountable to the Confederate States, on account of bounties or previums, pay, contingencies, arms or accoutrements, ammunition, stores, clothing, camp equipage, medicine, or medical instruments, or on any other account whatsoever.
