Hiram S. Fickle a member, of Captain Jno. F. McElhenny's Company (C), of the 37th Regiment of Confederate States Va. Vols., was enlisted by Capt John F. McElhenny, of the 37th Regiment of Va. Vols., at Lebanon on the 10th day of May 1861 to serve one year; he was born in Russell Co. in the State of Virginia, is 19 years of age, 5 feet 11 1/2 inches high, fair complexion, blue eyes, light hair, and by occupation when enlisted a Blacksmith. During the last Eight months said soldier has been unfit for duty ____ days.

I CERTIFY, that I have carefully examined the said Hiram S. Fickle of Captain McElhenny's Company, and find him incapable of performing the duties of a soldier because of injuries received in the left forearm from a muskett ball, rendering the hand almost useless.
