I CERTIFY, That the within named Fullin Hess a private of Captain Wm. J. Kendricks Company (G) of the thirty seventh Regiment of Virginia Infantry, born in Russell County in the State of Virginia, aged twenty years, four feet, eleven inches high, dark complexion, Black eyes, & Black hair, and by occupation a farmer, was enlisted by Wm. J. Kendrick at New Garden on the 13th day of July 1861, to serve one year, and is now entitled to discharge by reason of Rheumatism in both knee joints & enlargement of the Lymphatic Glands.

Private Fullen Hess of Captain Wm. J. Kendrick's Company of thirty seventh Regiment of the Confederate States was enlisted by Wm. J. Kendrick of the thirty seventh Regiment of Virginia at New Garden on the 13th day of July 1861 to serve one year he was born in Russell County in the State of Virginia is twenty years of age four feet eleven inches high complexion dark eyes and hair black and by occupation when enlisted a farmer during the last two months said soldier has been unfit for duty fifty days. he has been attacked with Rheumatism when just a youth & being exposed to cold in the camp in the latter part of the fall the disease returned on him the lymphatic glands become enlarged during this month.

I certify that I have carefully examined the said Fullen Hess of Captain William J. Kendricks Co (G) and find him incapable of performing the duties of a soldier, because of chronic Rheumatism which deprives him of the free use of both knee joints. He is also disposed to scrofulous inflammation and enlargement of the lymphatic glands.
