Henry P. Mead of Capt. John F. McElhenny's Co. "C" of the 37th Regt. Va Vols was enlisted by Capt. John F. McElhenny of the 37th Regt. Va Vols at Lebanon on the 10th day of May 1861 to serve for one year, he was born in Russell County in the State of Va. is 26 years of age 6 feet high fair complexion Hazel eyes yellow hair and by occupation when enlisted a Brick Mason. During the last 6 mos the said soldier has been unfit for duty.

I certify that I have carefully examined the said Henry P. Mead of Capt. Jno. F. McElhenneys Company (C), 37th Va. Regt. and find him incapable of performing the duties of a soldier, because of dropsy of the lower extremities, (Auasarca). He has been unable for service for near six months. I therefore recommend his discharge.
