Corporal Charles B. Price, of Captain Jno. W. Fickle Company, (C,) of the 37th Regiment of Confederate States Va. Vols. was enlisted by Capt. Jno. F. McElhenny, of the 37th Regiment of Va Vols at Lebanon Russell Co on the 10 day of May, 1861, to serve one year, he was born in Lebanon in the State of Virginia, is 20 years of age, 5 feet, 10 inches high, Dark complexion, Black eyes, Black hair, and by occupation when enlisted a Carpenter. During the last two months said soldier has been unfit for duty ___ days.

I CERTIFY, that I have carefully examined the said Charles B. Price of Captain Fickle's Company, and find him incapable of performing the duties of a soldier becuase of a gunshot wound of the right arm & which has produced partial anchylosis of elbow joint.
