CERTIFICATE OF DISABILITY FOR DISCHARGE.

Isaac N Cook a Private of Captain C. B. White's Company, (I) of the 16th Regiment of United States Ky Vols. Inft. was enlisted by Captain White of the ___ Regiment of ____ at Piketon, Ky on the 29th day of March, 1862, to serve three years; he was born in Russell Co. in the State of Virginia is Twenty Four years of age, 5 feet 9 1/4 inches high, Fair complexion, Black eyes, Dark hair, and by occupation when enlisted a Farmer. During the last two months said soldier has been unfit for duty 60 days.

He has been in this hospital 16 months without any benefit. Disease commenced in August 1862 while on duty.

I CERTIFY, that I have carefully examined the said Isaac N. Cook of Captain C. B. White's Company, and find him incapable of performing the duties of a soldier because of Paralysis of the extremities, completely disabling him. Cause unknown. Unfit for the Invalid Corps. Disability total.

TO BE GIVEN TO VOLUNTEERS AT THE TIME OF THEIR DISCHARGE TO ENABLE THEM TO RECEIVE THEIR PAY &c.

I Certify, on honor, that Isaac N. Cook, a Private of Captain C. B. White's Company (I) of the 16th Regiment of Infantry Volunteers, of the State of Kentucky, born in Russell Co. State of Virginia, aged 24 years, 5 feet 9 1/4 inches high, Fair complexion, Black eyes, Dark hair, and by occupation a Farmer, was enrolled or enlisted at Piketon, Ky on the 29th day of March 1862, and mustered in service by Lieut. Harris at Piketon Ky, on the 20th day of April, 1862, to serve in the Regiment for the term of three years; and having served honestly and faithfully with his Company in ____ to the present date, is now entitled to a discharge by reason of Paralysis of lower extremities completely disabling him from Disease commenced in August 1862.

The said Isaac N. Cook was last paid by Paymaster Maj. R. L. Wilson to include the 29th day of February 1864, and has pay due him from that time to the present date and he is entitles to pay and subsistence for traveling to place of enrollment and whatever other allowances are authorized to volunteer soldiers, or militia, so discharged.

He has received Clothing from the United States, not paid for, amounting to Forty Three and 04/100 Dollars.

He is entitled to Clothing page from the Twenty Ninth day of March, 1862, to the time of discharge.

Given in duplicate at U. S. Gen'l Hospital Quincy Ill this Twenty Seventh day of May, 1864.
